貴社・貴グループで購入するラッフル券のセット総数と合計額を入れてください。

万が一イベントを延期せざるを得ない場合は、4月16日（日）に延期となります。お支払い額は、返金不可です。
Should the picnic be cancelled for some reason, it will be rescheduled on 4/16/23. The payment is NOT refundable.

＊ラッフル券は、会社・グループごとに代表者が事前に三水会センターにてお受け取り下さい。やむを得ずピクニック当日に受け取る場合は、本部（King Pavilion)にてお受け取りください。
Please have your company/group representative pick up the raffle tickets at Sansuikai Center prior to the event. If for some reason tickets cannot be picked up in advance, they can be picked up at the JBAH booth(King Pavilion) of the Oil Ranch on the event day.


ラッフル券の三水会センターでのお引渡しは、通常お申込みから3日後よりとなります。事務局よりメールにてご連絡を致しますのでお引き取り日時をご確認下さい。
